What counts as a correction

A rate we reported that was never accurate, a source we misread, a figure attributed to the wrong vendor, or a claim that outran its evidence. Routine updates, where a vendor changed a published rate and we refreshed the row, are recorded in the dataset's verification dates rather than here.
